When you use our contact form, we collect your name, phone number, email address, the best day and time to reach you, whether you are a new or existing patient, and your message.
We use this information only to respond to your request. The form is sent to our office by email through Resend, an email delivery service.
Please do not send Social Security numbers or other sensitive information through the contact form or by email.
