Filling out your information before your first visit can save you time at our office. Print the forms below, fill them out, and bring them to your appointment.
If you would rather not print them, please be ready to fill out insurance and health information forms when you arrive.
What each form covers
The patient registration form asks for your contact details, an emergency contact, and your dental insurance information. Please bring your current dental insurance card and your driver's license to your appointments.
The medical history form asks about your health, medications, and allergies. Your mouth is part of your whole body, and health problems or medications can be closely related to the dental care you receive.
The dental history form asks about your past dental care, any symptoms you have noticed, and any concerns you would like your dentist to know about.
Privacy and consent forms
The consent form explains how your health records are used for your care, for billing, and for our office's operations.
The acknowledgment form confirms that you received a copy of our Notice of Privacy Practices. The form notes that you may refuse to sign it.
